Job Requirements

Qualifications:

Experience:

• Proven experience in a similar role
• Experience in hospital case management, discharge planning, or related medical field beneficial
• Knowledge of external private and government frail care services and home care services
• Familiarity with the Older Persons Act

Key Responsibilities

• Coordinate and manage the discharge planning process at Tokai Sub-Acute
• Evaluate patients’ needs for individualized discharge plans
• Collaborate with healthcare professionals
• Educate patients and families about discharge processes
• Advocate for patients and report suspicions of abuse or neglect
• Participate in quality improvement initiatives
