Experience:

- Experience in hospital case management, discharge planning, or related medical field beneficial
- Knowledge of external private and government frail care services and home care services
- Familiarity with the Older Persons Act

Key Responsibilities
- Coordinate and manage the discharge planning process at Tokai Sub-Acute
- Evaluate patients’ needs for individualized discharge plans
- Collaborate with healthcare professionals
- Educate patients and families about discharge processes
- Advocate for patients and report suspicions of abuse or neglect
- Participate in quality improvement initiatives
